My red ear slider is a male around two years old I feed him reptomin pellets and he has a heat lamp and I use only reverse osmosis water in his tank and he has a great filter but I feel like his back shell looks unhealthy

He has a lot of retained scutes. You can even see his "baby" scutes there and the overlapping scutes might cause a problem. Most likely he's been over fed a bit, though I don't see any significant pyramiding - which is good. Make sure he basks regularly, has access to uvb, and his diet has both calcium and vitamin d3.
