RES Flipping Over On Shelf-PLEASE HELP

We have a 18 year old RES in a 175 gallon tank with a ramp that leads to a sun shelf.

Lately he has been climbing up on his back legs trying to escape and flipping over on his back-shell.

This can prove to be fatal if not caught rather quickly.

Does anyone have any suggestions on how to get him to stop doing this?

Can you post a picture of the basking area? They should be able to flip themselves back if on a hard, flat surface.

It's a flat, large area so he should able to flip himself back. As for stopping his from wanting to climb out, not sure if there is much you can do. The smooth areas should be helpful in preventing him from doing much damage.
