My RES has been fine for months but just yesterday i saw him laying on the driftwood before I left my house and when I returned he was still there, and same in the morning. he seems pretty lethargic and isn't eating. Barely moving around the tank, is this a sign of RI?

It's normal for them to be occasionally moody but if you feel something is off, you should have him seen by a vet. Does he seem weak or just not interested in the usual turtle things?

hes generally energetic and runs swim around everywhere but seems weak now, I did a carrot soak and he was more active afterwords but still not eating

I would give him another day or two but do some research on a suitable vet in your area.
