Help!!! Terrapin expanding???

Hello guys, i am not sure what the heck is happening, but my terrapin's side thingy is expanding. Here is a photo. I thought he was just getting fat, but it became so big n the otherside is normal sized. OMG HELP.

It could be organ failure or an infection. I would take him to a vet. From the looks of the shell, he has a poor diet and environment.

Yes, it is serious. As for the setup... too small, not enough water, no UVB lights + heater, inadequate filter + basking area. The difference is sizes with the turtles is not good either.
