What does he normally eat? What's his basking/UVB light setup like and has it changed recently?

He normally eats the Taiyo Turtle food and is not interested in other vegitables or fishes.

I have set up the tank outside with an area to bask getting sunlight. It have never been changed .

His eyes are always closed these days and is so inactive.

Barely walked some distance today.

He puts his head into water sometimes and stays on the basking stone itself

Dont put him in tank outside, he should have a safe and secure place to roam around if possible. I would try applying some cod-liver oil to his eyes. Do you have any pictures of him?

Its a shady area and safe also have ample space to swim and an area to bask also. Will take the latest picture and upload . He is not opening his mouth also and is always on the stone and not eating or opening eyes . Consulted a vet and they gave antibiotics for him to be fed 2 drops daily but he is not even opening mouth so i cant feed !!!

Ordered This online in case it does any good !! :
Pet Care International (PCI) Turtle i-Care & Turtle VIT Drop for Swollen, Inflamed, Infected Eye & for Essential Vitamins and Minerals of Healthy Turtle Healthcare (Combo) (30ml)

From the picture, it looks like you have a girl. Most turtles won't eat if they cannot see. The vet did not offer an antibiotic ointment?

No they haven't , antibiotics drops was what they gave and I'm unable to feed it too as it's not opening mouth at all.

Anyway or what is the treatment for this ? Why is it always inactive and eyes closed ??

Without being there to examine your turtle, I could only throw out guesses. It's usually an injury, vitamin deficiency or an environmental factor. Maybe soak the turtle in some warm water in a shallow container and add some unflavored Pedialyte and crumbled up pellets for her to ingest.

It passed away today  She herself went into the water i was able to feed her 2drops of the medicine . She stayed under the water at night and one side of her body was slightly floating

In the morning I saw it laying motion less and had passed away.

I examined her body but couldn't find any external injury or anything..

I'm very sorry to hear it. RES are very good at hiding their illnesses, you can't tell how long she was sick.
