Urgent Care :: My RES turtle isn't eating or moving much

My 5 month old RES doesn't eat or move much. He just stays at the bottom of the tank. However, when I nudge him, he starts moving and swimming.

He will continue to decline if he doesn't eat. Without some of the other symptoms present, it's hard to understand whats going on.
